ADJUVANT MEDICAL CANNABIS IN SEVERE TRAUMATIC BRAIN INJURY: FUNCTIONAL AND NEUROLOGICAL OUTCOMES

Abstract

Traumatic brain injury (TBI) is a major cause of long-term neurological impairment, often resulting in persistent motor, cognitive, and behavioral deficits. The management of severe cases typically requires multimodal therapeutic approaches, with growing interest in adjunctive therapies such as medical cannabis. We report the case of a previously healthy 28-year-old female who sustained severe TBI following a motor vehicle accident. Her clinical course was marked by prolonged intensive care unit admission, respiratory complications, and significant neurological impairment, including spastic hemiparesis, cognitive deficits, and functional dependence. Medical cannabis was introduced during hospitalization and subsequently administered through systemic, topical, and inhalational routes throughout the rehabilitation process. Over a five-year follow-up period, the patient demonstrated progressive clinical and functional improvement, evolving from a bedridden state to assisted ambulation with crutches. Notable outcomes included improved cognitive engagement, reduction in seizure episodes, decreased sialorrhea, enhanced emotional stability, and accelerated healing of pressure ulcers following topical cannabis use. This case highlights the potential role of medical cannabis as an adjunctive therapy in the rehabilitation of severe TBI, particularly in addressing complex neurological and functional impairments. The multimodal use of cannabis may contribute to symptom control and functional recovery, supporting its consideration in integrative therapeutic strategies.
