RELAÇÃO ENTRE A DOSE INGERIDA DE PARACETAMOL E O NÍVEL SÉRICO EM INTOXICAÇÕES AGUDAS: ESTUDO TRANSVERSAL EM UM CENTRO DE REFERÊNCIA BRASILEIRO

Introdução: a dosagem sérica de paracetamol entre 4 e 24 horas após a ingestão, interpretada pelo nomograma de Rumack-Matthew, é o padrão-ouro para indicar N-acetilcisteína (NAC) na intoxicação aguda. Onde esse exame não está disponível em tempo hábil, a dose relatada pelo paciente costuma substituir o critério laboratorial, apesar de evidência internacional limitada e heterogênea sobre sua validade preditiva. Objetivo: avaliar a associação entre a dose de paracetamol referida na admissão e o nível sérico coletado entre 4 e 24 horas após a ingestão, e descrever o perfil clínico-epidemiológico dos casos. Métodos: estudo transversal, com coleta prospectiva de dados, conduzido no Centro de Toxicologia do Hospital João XXIII (CIATox-MG), Belo Horizonte, entre maio de 2022 e dezembro de 2023. Foram elegíveis pacientes com ingestão aguda (<8h), dose relatada ≥10 g ou ≥200 mg/kg (o menor valor) e coleta sérica na janela de 4–24h. Toxicidade foi definida por resultado acima da linha de tratamento de 150 mg/L no nomograma de Rumack-Matthew. Variáveis quantitativas (medianas, Q1–Q3) foram comparadas por Mann-Whitney; variáveis categóricas, por qui-quadrado; α = 0,05. Resultados: de 1.223 atendimentos por paracetamol no período, 41 pacientes preencheram os critérios de inclusão. A amostra foi predominantemente feminina (70,7%), com mediana de idade de 24 anos (Q1–Q3 21–31) e dose ingerida de 15 g (11,4–26,4). Todos os casos decorreram de tentativa de autoextermínio. O nível sérico foi tóxico em 51,2% da amostra. Não houve associação entre toxicidade e dose ingerida em gramas (p = 0,636) ou em mg/kg (p = 0,375). O intervalo entre ingestão e coleta (delta T) foi maior no grupo tóxico (mediana 8h vs. 6h; p = 0,023). Nenhuma variável categórica analisada (sexo, faixa etária, coingestão, procedimentos de descontaminação) associou-se à toxicidade. Conclusão: neste estudo exploratório de amostra pequena, a dose autorreferida não se comportou como preditor de toxicidade hepática, enquanto o tempo até a coleta sérica sim. Os achados são consistentes com a manutenção da dosagem sérica como critério preferencial de indicação terapêutica, mesmo em contextos de recursos limitados, e não sustentam, por si só, qualquer mudança de protocolo clínico.
