MANEJO MULTIDISCIPLINAR E DESAFIOS TERAPEUTICOS NO TRATAMENTO DA DEMENCIA COM CORPOS DE LEWY

Introdução/Objetivo: A demência com corpos de Lewy (DCL) é a segunda principal causa de demência neurodegenerativa, mas que permanece subdiagnosticada devido à sobreposição clínica com a doença de Alzheimer e a doença de Parkinson. Este estudo objetivou analisar as evidências recentes sobre a fisiopatologia, o manejo multidisciplinar e os desafios terapêuticos da DCL. Métodos: Trata-se de uma revisão narrativa da literatura, conduzida na base PubMed (2021-2024), utilizando os descritores MeSH “Lewy Body Disease” e “Therapeutics”. Foram incluídos estudos originais, revisões sistemáticas e ensaios clínicos que abordassem mecanismos moleculares, reposicionamento farmacológico ou intervenções não farmacológicas. Resultados: A fisiopatologia envolve agregação de α-sinucleína, disfunção mitocondrial (com redução da respiração máxima e da capacidade glicolítica), falência autofágica (via SNAP29) e infiltração de linfócitos T CD4+ mediada pelo eixo CXCR4-CXCL12. Ensaios de triagem por RT-QuIC identificaram quatro fármacos aprovados (entacapona, tolcapona, fenazopiridina e zalcitabina) com potencial de reposicionamento antiagregação. Ensaios recentes de amplificação de sementes de α-sinucleína em líquido cefalorraquidiano e biópsia cutânea alcançam sensibilidade próxima de 95% e especificidade em torno de 97%, sinalizando uma virada de chave no reconhecimento precoce da doença (Concha-Marambio et al., 2025; Wang et al., 2025). No campo terapêutico, os achados de fase 2b do ensaio RewinD-LB com neflamapimode e a análise agrupada do HARMONY, com pimavanserina, ampliam o horizonte para intervenções mais seguras em domínios historicamente refratários (Prins et al., 2024; Ballard et al., 2025). Intervenções não farmacológicas, sobretudo fisioterapia motora e reabilitação cognitiva, demonstraram benefícios sobre marcha e sintomas neuropsiquiátricos, ainda que a qualidade metodológica dos estudos permaneça predominantemente baixa. Conclusão: O cuidado na DCL exige uma estrutura multiprofissional, integrando terapias farmacológicas criteriosas (com rigorosa vigilância para neurolépticos) e não farmacológicas, além de estreita articulação entre a pesquisa translacional e a prática clínica.
